Broadband Ohio Internet Access Survey

Thank you for participating in the BroadbandOhio Internet Access Survey. The State of Ohio is eligible for a

significant amount of federal funds to improve access to high-speed internet. To receive these funds, we need
input from Ohio residents like you on the state of broadband in your communities. After completing the survey,
you can use the stamped envelope provided to return the survey to BroadbandOhio free of charge.

This survey does not request or seek to collect any personal identifying information, and we ask you not provide
any in any of the open responses. Participation is voluntary and you may stop answering questions at any point.

BroadbandOhio is an office of the Ohio Department of Development, a public entity subject to public records
laws, including Ohio Revised Code Section 149.43. BroadbandOhio is required to maintain many things
submitted to it as public record, which are generally open to the public for inspection. Unless otherwise exempt
under applicable law, any information submitted in this form may be disclosed in response to a public record
request.
